Patient
STEPS TO START THE HOME HEALTH ASSISTANCE SERVICE WITH THE PATIENT:
If a patient, or a family member, wishes to obtain home health aide services through our agency, there is only one thing they need to do.
The patient must have been PREVIOUSLY approved with hours of service through the LONG TERM CARE (Alliance for Aging) program.
Once this service is approved, the patient just needs to provide our information to the case manager assigned by the insurance. In fact, patients and family members are always welcome to contact us directly with any questions or concerns.
We will be able to coordinate the service according to the patient’s needs, as to the appropriate schedule, the details of each case they need help with, and overall preferences.
